How does Sculptra work? Collagen neosynthesis via foreign body response

Poly-L-lactic acid (PLLA) is a synthetic, biodegradable polymer — biocompatible, non-immunogenic. Sculptra's mechanism of action is fundamentally different from HA fillers:

• Phase 1 (weeks 1–4): PLLA microspheres initiate a controlled foreign body response — fibroblasts surround the particles and begin producing new collagen,
• Phase 2 (months 2–6): Effect gradually increases — skin becomes denser, volume restored, lifting visible,
• Phase 3 (months 6–18): PLLA biodegrades — but the collagen produced by fibroblasts remains,
• Final result: lasts up to 2–3 years — significantly longer than any HA filler or CaHA biostimulator.

Sculptra does not fill — it stimulates the skin to produce its own collagen. The effect is gradual, natural and long-lasting.

How does Sculptra differ from other tissue stimulators (CaHA, PDRN)?

Sculptra (PLLA) stimulates collagen via a controlled foreign body response — fibroblasts produce collagen in response to PLLA microsphere presence. CaHA stimulates fibroblasts through biochemical pathways (biochemical receptors). PDRN acts via adenosine A2A receptors. Sculptra's effect is the most durable (up to 3 years) — the produced collagen persists long after PLLA biodegrades. Effects accumulate gradually over months, making Sculptra ideal for patients seeking natural results without dramatic and immediate change.

How many sessions does Sculptra require and when do effects appear?

The standard protocol is 3 sessions at 4–6 week intervals. Effects are NOT immediate — Sculptra does not work like a filler. First visible results typically appear 4–6 weeks after the first session, full effect after the series is complete. Patients must be informed about the gradual nature of results.
